What is PDO (Participant Direction Option)?
It is a model of care that allows Medicaid recipients with disabilities to direct their own home and community-based services (HCBS), instead of receiving them through a traditional agency-managed model.
The Designated Representative role in the Participant Direction Option program is a voluntary, unpaid position. The DR is appointed to assist the participant with managing their services, such as hiring caregivers and completing necessary paperwork. This role is not considered a paid caregiver or employee position.
Under Florida law and Medicaid rules, anyone involved in providing or managing long-term care services—including DSWs and DRs—must undergo a Level 2 background screening, which includes a fingerprint check through the Florida Department of Law Enforcement (FDLE) and the FBI. Background screenings must be redone every five years.
Your Designated Representative cannot also serve as your paid caregiver (DSW). This is to avoid conflicts of interest and ensure accountability in managing services. The DR is responsible for tasks like hiring, supervising, and approving timesheets, which would be a conflict if they were also receiving payment.
In Florida’s Participant Direction Option (PDO) program, you do not need a legal guardian unless you have been formally declared incapacitated, and the court has appointed one. A legal guardian is only required when a person is legally found unable to make decisions for themselves.
No, a legal guardian and a Power of Attorney (POA) are not the same, although both can make decisions on someone else’s behalf.
  • A legal guardian is appointed by a court to make decisions for a person who is legally determined to be incapacitated or unable to manage their own affairs. This role comes with court oversight and specific legal responsibilities.
  • A Power of Attorney (POA) is someone chosen voluntarily by an individual (while they are still mentally capable) to act on their behalf for financial, medical, or other matters. It does not require court involvement and can be limited or broad depending on the document.
No. PDO services must be provided within the state of Florida. If you plan to travel or temporarily relocate, you should notify your managed care plan and case manager.

No. PDO services are intended to support participants in their own homes or community settings. When a participant is admitted to a hospital, Adult Day Care, Assisted Living Facility, or other institutional setting, PDO services cannot be provided.

During such stays, care is provided by the facility. You should notify the managed care plan and Case Manager if the participant is hospitalized or admitted to another care setting to adjust services accordingly.

Your hours and services are based on your individual needs as assessed by your health plan. This assessment considers your health condition, daily living activities, and goals.

Your Case Manager develops a care plan that outlines the types and number of services you qualify for, including how many hours of care you can receive. Your approved hours and services are then authorized and managed through the PDO program to ensure you get the support needed to live as independently as possible.

To manage your services effectively in the Participant Direction Option (PDO), you’ll need access to a few key technologies. You need a smartphone, tablet, or computer with reliable internet access. Additionally, you will need a basic email account, which is needed to register for portals and receive login instructions.
